Financial Aid will not be considered unless this form is completed with a copy of the sports registration form. An application must be filled out for each sport, each year.

Applications from individuals or organizations who apply for K-BALP funding will be reviewed and approved by the K-BALP Team.  Approval will be based on meeting household income requirements, and submission of proper supporting documentation.  Once documents have been reviewed and approved, you will receive an email of acceptance and funds will be distributed accordingly.
